Fishbone Diagrams

What is a Fishbone Diagram?

A Fishbone Diagram is a structured brainstorming tool designed to assist improvement teams in coming up with potential root causes for an undesirable effect. Its name derives from its resemblance to the bones of a fish. It is also known as a Cause and Effect Diagram or an Ishikawa Diagram after its creator. 

KaiNexus now allows you to easily create fishbone diagrams in the same place you manage the improvement work to address the problem.  



Where can I view a Fishbone Diagram?

NOTE: Fishbone Diagrams will need to be enabled for your Template(s). Reach out to your Customer Experience Lead to take advantage of this feature.

 

You can add a Fishbone Diagram to be visible while working directly with your Items. Depending on the layout of your Template(s) will determine how the Fishbone Diagram appears within your Items.

 

For Standard Layout Templates, you can view the Fishbone Diagram within its own panel underneath the Details panel within your Item. 

NOTE: If Resolution has been enabled on your Template, the Fishbone panel will appear underneath the Resolution panel.

By default, a new “Fishbone” tab will also be added on Standard Layout Templates where users can view and interact with the Fishbone Diagram as well.

NOTE: This “Fishbone” tab can be configured to be hidden as well. Reach out to your Customer Experience Lead to help with this configuration.

For Custom Layout Templates, you have the option to include the Fishbone Diagram at the bottom within any one of your “Details” panels or you can have the Fishbone Diagram appear in its own custom “Fishbone” panel.

Who can add a Fishbone Diagram to an Item?

 

Anyone who has permission to edit the Item has the ability to add and edit a Fishbone Diagram within the Item.

 

People who can work with the Fishbone Diagram:

 

Projects:

Improvements:

 

Incidents:

Tasks:

 

Charts:

  • Author(s)

  • Anyone with the “Edit - Charts” permission if the Chart is in the specified Location(s).




How to add a Fishbone Diagram to an Item?

 

To add a Fishbone Diagram to your Item:

  • Open up the Item you are working with.

  • Click the Plus_in_box.png in the top right corner of the Item.

  • In the resulting drop-down menu, click Fishbone.png.

In the resulting window, you can:

  • Define the undesirable effect in the “Problem” field. This is shown as the “fish’s head” to the far right on the diagram.

  • Enter in titles for each Category that will help group each list of potential causes for the defect listed above.

NOTE: You can have up to six Categories on your Fishbone Diagram, however, you are not required to use all six.

  • Underneath each category, you have the option to enter in free-text or search for existing Items within the system to add to your diagram.
    • To add a line of free-text, click the Plus_gray.png next to the “Start typing…” drop-down and in the newly formed row, type the text you would like to appear in the diagram under the respective Category.

    • To add a link to an existing Item, click in the “Start typing…” drop-down either the Title or the ID # of the Item you are looking for and select it within the resulting drop-down to have it appear in the diagram under the respective Category.

NOTE: This will provide a direct link to the Item when viewing the Fishbone Diagram.

 

  • Repeat these steps above for as many times as needed till you are satisfied with your Fishbone Diagram.

  • When you’re ready, click Save.png.



The resulting Fishbone Diagram will be displayed within your Item.

How to edit a Fishbone Diagram on an Item?

 

To edit an existing Fishbone Diagram:

 

EITHER:

  • Open the Item with your Fishbone Diagram.

  • Click the EllipsesButton.png in the top right corner of the Item.

  • In the resulting drop-down menu, click Edit_Fishbone.png.

OR: 

  • Open the Item with your Fishbone Diagram.

  • Navigate to the panel that contains the Fishbone Diagram.

  • Click the Screen_Shot_2021-06-23_at_12.52.42_PM.png icon directly on the Fishbone Diagram to add a Category, Cause, or Problem.

  • Alternately, you can also click the Edit.pngnear the top right of the panel.

In the resulting window, you can:

  • Edit the undesirable effect in the “Problem” field. This is shown as the “fish’s head” to the far right on the diagram.

  • Repeat these steps above for as many times as needed till you are satisfied with your Fishbone Diagram.

  • When you’re done making edits, click Save.png.



Working with your Fishbone Diagram

 

If you would like to make any edits to just a certain section of the Fishbone Diagram you can by clicking directly on the Category Title.

 

Doing this will allow you to edit both the Title of the respective Category and any of the potential causes listed underneath the Category on the Diagram.

You can also edit the undesirable effect by clicking on the “fish’s head” along the far right of the Diagram.

Aside from making edits to the contents of your Fishbone Diagram, you have the ability to both highlight and/or strikethrough any of the causes that have been listed on your diagram to help give a better visual understanding of the Fishbone Diagram.

 

To highlight a cause:

  • Hover over the cause on your Fishbone Diagram.

  • Right-click on the cause you wish to edit.

  • In the resulting drop-down menu, click Highlight.png.

To remove a highlight from a cause:

  • Hover over the highlighted cause on your Fishbone Diagram.

  • Right-click on the cause you wish to edit.

  • In the resulting drop-down menu, click Remove_Highlight.png.

To strikethrough a cause:

  • Hover over the cause on your Fishbone Diagram.

  • Right-click on the cause you wish to edit.

  • In the resulting drop-down menu, click Strikethrough.png.

To remove a strikethrough from a cause:

  • Hover over the strikethrough cause on your Fishbone Diagram.

  • Right-click on the cause you wish to edit.

  • In the resulting drop-down menu, click Remove_Strikethrough.png.

Click ExpandPanelIcon.png to bring up the Fishbone Diagram within a separate window for easier visibility.

Click Notifications_expand.png to expand the Fishbone Diagram to take up your entire screen.

How to remove a Fishbone Diagram from an Item?

To remove a Fishbone Diagram:

 

EITHER:

  • Open the Item with your Fishbone Diagram.

  • Click the EllipsesButton.png in the top right corner of the Item.

  • In the resulting drop-down menu, click Edit_Fishbone.png.

OR:

  • Open the Item with your Fishbone Diagram.

  • Navigate to the panel that contains the Fishbone Diagram.

  • Click the Edit.pngnear the top right of the panel.

In the resulting window:

 

  • Click the Delete_Fishbone.png in the bottom right corner.

  • In the following window, make sure you check the necessary box. 

NOTE: This action cannot be undone.

  • When you’re ready, click Delete__blue_.png.
